I shall first up the moat malignant form, as being In its the most important, and as embodying, moat ful ly, in its details of treatment, the great Mimi plea of cure which are alike 'applicable to. all forms, of the disease. All the modifications of the disease require to be treated on the same priaciples, lhe only difference being that, in the detail, the milder forms require less powerful doses of the medicines. The mode of treating the most malignant form of the disease will serve as the model on, which all the others are to be treated. The moat malignant form has, by all, writers on the, subject, hitherto, - been pro: nonuced iecurable. They say it neverwas cured in a single instance. I aludl, however, point out a mode of treating it which will prove itself in falliblisuccessful, where my directions are fol. lowed with sufficient promptness, boldnetw, and Skill. I would recapitulate, that the symptoms in this case are great languor and depression of spirits; giddiness of the head; soft, small varia ble pulse, tongue cold, iloWing. with 'saliva, re laxed. broad, and tretaulotec-heat of skin below the natural temperature; no cramps or pains, but an indescribable feelingof anxiety and crush ing about the heart; accompanied with watery purging and vomiting, or with watery pargiag All - these symptoms indicate the utmost degree of malignity, and not ouilsomezit is to be. lost in the vigorous applicatiois of the most powerful remedies.'The - disease in this form rune its amine so rapidly, that before the medical atten dant arrives, it may have so far progressedsthat one additional discharge from the bowels May carry the patient into hopeless collapse. `'l i ne practice, therefore, meat be prompt. The 'dia.;, charges front the owels must be stopped at once , and for this purpose an efficient dose of Medicine must at once be administered. Tritlesvithanet , ficient first done and the patient is lost; rultoinis tered with the Ixddness I shall prescribe, success is as certain as is the relation between cause and effect. •. Place the patient Immediately in the horizon.' talposture in bed, and:give bite, mettle instant' .as Wits an extreme case, ,ten of the .anti-spas tialskluvitis, and two-ounces of the above anti wpainCodie mixture, and wash the whole, down with • gists. undated brandy or 'whiskey,. Oa enStd strongly with cloves, essence of ginger, or some such .warm aromatic spice. In We mean time have. him covered - with an additional blanket and let the usual means of communicating heat such as jets or bottles of hoe water, bags of hot salt or sand, hot bricks , or whatever can be most readily procured, be applied without delay, to the feet, mud different parts oi the body, so as to I restore the temperature, and preduce perspir tion as quickly as possible.' AS soon its the per..., spiratlon has begun to flow freely, superadded to the medicine and cordials already administer ed, a glass of brandy patch should be given— the: punch to be made strong, and to be au-allow ed hot as possible. After this no driok should be given till the perspiration has flowed freely form few minutes. The stomach will then re tain it, and the patient should be indulged free ly with copioua draughts of rennet whey, warm toast water,-ffimmred with some agreeable spice, mint or balm tea ; or swatch fund beventee. The necessity of attending to this is most huger - tent- When the discharges -from the bowels' cease, and when the pulse becomes full and be end: ing—the body is covered with a copious . warm perspiration ' which will not fail to be the - , cane under such treatment, the danger is over. The perspiration, if the patientcan bear it, should be kept up for twelve hours 1 and may with advan tage be continued, moderately, even longer.— its duration, however, must be regulated accor ding to the strength of. the patient and the state of the pulse.' After the first four or six hours, more heat need not be applied - than is perfectly agreeable to the feelings of . the - patient. It ie remarkable how suddenly the praccordial- op pression, are relieved on the breaking out of o free perspiration: and, whet is of greater importance still, the Vomiting, where it exists, immediately ceases. I know of no other means by which vomiting irCeuch cases can be speedi ly and offeetually, checked. In tho' ipplicatlon of external heat, a rational use should be made , of , the means. I cannot see the 'necessity for .increasing the temperature beyond what isgrate-, ' ful to the feelings of the patient, and heyond what is sufficient to Piduce and keep up a pro . . fuse perspiratitm.": ,1 would remark, • that the heat cad be much more efficiently communicated by solld tmbstances, . such as' I have mentioned alUsie; than•by the hot air or :vapor apparatus. This apparatus, as a meson communicating heat to a patient affected with 'cholera, is an in strument which . I colander to bo 'worse-. than useless. . Now, let it be observed, that I hare selected an extreme case, and have prescribed a dose of medicine sufficient to meet such a case. As I have already stated, not one unto need be lost if the practice be sulfieleotly prompt awl bold. I hive supposed a ale of the most malignant character, when there has been •profuse watery parging, and where another discharge from the bowels would endanger the patient's life; and, under these circumstance., I have prescribed ten of the pills containing ten grains of powdered opium, as a less dose would .not meet the exi gencies of the case. To administer- lids dose, under the circumstances !bile stated, is per fectly safe, to administer an inefficient dose Is certain death. I have, under the circumstances supposed, tried smaller doies, bat found them insufficient to arrest the progress of the symp toms, and wee obliged in a few minutes to in crease them. After such orperience I always prescribed ten of the pills, for a dose, under the alarming and dangerous circumstances I have supposed, and always with never-failing eta:l een ; and I have never seen the alighteit narco tic effect produced by this largo dose of medi cine, on any of the patients to whom it was ad ministered under such circumstances. The rea son why such a large dose of opium mbe safely administered In 'lnch case, and the e reason why, under such circumstances, it is absolutely necessary, I have fully explained in the prime , ding letter. • Should there, however, have been little or DO I purging, a smaller dose of the pilismust be given The system not having sufferrerl much depletion from the escape of the serous part of the blood, eo vary large a dose of opium is not necessary. In snob cat* eight of the pills will generally be .utgalent,—to be - accompanied, however,- with the same quantity of the antlepumodiemixture, andthe nab cordial stimulants aa already pre scribed, and to be followed with equal prompt ness by exciting the prespiration, • that grand ngcnt in the cure of of every modification of Cholera, without Wideli a: malignant case of the disease could not, by possibility, be cured.
